Key differences
Without premium: Welcome screen shows I feel sick / I need general information / I don't know where to start / Call a nurse (1177). No premium options visible. With premium: Same welcome screen gains "Unlock premium features" button in amber. After unlock, premium options appear across all relevant screens. The distinction is designed to be visible but not intrusive — the free tier is fully functional on its own.
